Changed Z_PROBE_AND_ENDSTOP to Z_PROBE_ENDSTOP.

Updated documentation in Configuration.h.
Cleaned up and commented some code relating to Z_PROBE_ENDSTOP.
Separated Z_MIN_ENDSTOP and Z_PROBE_ENDSTOP completely.
